Required Forms on CampDoc
Participants and parents will receive required forms through a software called CampDoc. All forms in CampDoc are required. When forms are ready to sign, participants and parents will receive an email. Forms will be due through the CampDoc website on Monday, June 15, 2026.
Health Insurance

Go to Get Your Aetna ID Card. You’ll enter the following, after which you will receive your insurance card in PDF form via email:
- USC Student ID number
- Date of birth
- USC email address — this is your Net ID plus @usc.edu (i.e., trojan@usc.edu).

International summer programs participants are automatically enrolled in the Aetna Student Health Insurance Plan (SHIP), regardless of their current plan.
Vaccinations

Yes, all USC Summer Programs participants (domestic and international) are required to meet the same immunization standards as USC’s general student population.

The following vaccinations are required:
- MMR (measles, mumps and rubella): Submit proof of two doses of the MMR vaccine, or obtain titers, a blood test to verify immunity to measles and mumps.
- Tdap (tetanus, diphtheria and pertussis, or whooping cough): One dose within the past 10 years.
- Varicella (chickenpox): Submit proof of two doses of the varicella vaccine, or obtain titers, a blood test to verify immunity to chickenpox.
- If you had measles, mumps or varicella as a child, or if you are unable to obtain proof of vaccination, you must obtain a blood titer test.
- Meningococcal ACWY: Submit proof of two doses of the meningococcal vaccine. For participants under 16, the most recent dose must be within the 11th or 12th year. If you are 16 to 20 years old, the most recent dose must be on or after your 16th birthday.
- Hepatitis B: Two-dose or three-dose series. For participants under 18, three doses are required. For participants older than 18 who received Heplisav-B, two doses are required.
- Polio: Four-dose series.
- Tuberculosis screening: This applies only to international participants who are citizens of countries designated by the World Health Organization as having a high TB burden.

The following vaccinations are strongly recommended but not required:
- Hepatitis A vaccine: Two-dose series.
- HPV (human papillomavirus) vaccine (Gardasil 9): Three-dose series (recommended for all students, regardless of gender, through age 26).
- Meningococcal B vaccine (Trumenba, Bexsero or Penbraya): Two- or three-dose series (recommended for ages 16-23).
- COVID-19 vaccination.
- Polio vaccine.
- Tdap (tetanus, diphtheria and pertussis).

All records must be uploaded through the CampDoc portal by the program deadline. Records must show your name and date of birth in order to be accepted. Screenshots for health portal records are accepted. For records in languages other than English, translations into English must be provided.

Yes, religious/philosophical and medical exemptions are allowed.
Students with a medical condition that is a CDC contraindication to vaccination for the following vaccines will need to have their medical provider complete the Medical Exemption form and submit to USC:
- MMR
- Varicella
- Meningococcal conjugate

Yes, as long as each dosage was on or after your first birthday and you received two dosages of each separated by at least 28 days.

You must receive a third dosage. All doses must be on or after your first birthday.

No. Even if you had chickenpox as a child, you must obtain a titer, a blood test, to verify immunity.

Yes. If the titer is positive, that will satisfy the varicella immunity requirement. If the titer is negative, documentation of two doses is required.

No. You must receive a dose of Menactra or Menveo (meningococcal ACWY/MCV4) on or after age 16.

No. We only accept documentation of the vaccine on or after the age of 16.

Yes. Hepatitis vaccines are generally considered to provide lifelong protection in healthy individuals.

Yes, if they clearly show vaccine names and dates. English translations are required.

No vaccines will be administered for missing vaccine requirements. Tdap will be given for any scenarios involving injuries with a break in the skin.

Submit available records and notify the program. Cases are often reviewed individually.
Tuberculosis Screening

International students with citizenship from “high burden” countries, as designated by the World Health Organization, are required to submit a laboratory report of a negative Quantiferon or Tspot blood test completed in the United States or Canada upon arrival to USC.
If you have an tuberculosis appointment included with your check-in materials, you are required to attend the appointment and complete the tuberculosis screening. The screening is not mandatory for domestic students.

Additional evaluation (e.g., chest X-ray or doctor’s note) may be required.

You must submit proof of the positive result and a normal/negative chest X-ray report done within six months prior to entry to USC to confirm that you do not have active tuberculosis.
X-ray images will not be accepted; the report must be submitted.
