USC SUMMER PROGRAMS - REQUIRED FORMS

Below are the required forms that you will need to complete with a parent or guardian prior to your arrival on campus. In late April or May, you will receive an email from CampDoc with an invitation to log into your existing Camp Doc profile or to claim a new profile. Once logged in, you will need to complete the following forms:

Medical Authorization
Medical information that is required in case of an admission to the USC Health Center.

General Information

Emergency Contacts
You will be required to share contact information of individuals who should be contacted in the case of an emergency.

Allergies
You will be required to share any allergies of which program administrators should be aware.

Medications
You will be required to share any medications of which program administrators should be aware.

Immunizations
You will be required to show immunizations for the following:

  • MMR (measles, mumps, rubella) – 2 doses
  • Varicella (Chicken Pox) – 2 doses
  • Tdap (tetanus, diphtheria, pertussis) – 2 doses
  • Meningococcal – 1-2 doses (if older than 16, must show two doses with the most recent being on or after the 16th birthday)
  • Hepatitis B – 3 doses
  • Polio – 4 doses
  • Tuberculosis
    • The TB test must:
      • take place in the United States or Canada
      • be a Quantiferon or T-Spot blood test with a completed laboratory report
      • take place within the 6 months before arriving on campus; any test performed more than 6 months from your class start date will not be accepted
    • International students do not need to show TB testing as they will be required to take a TB test on campus upon arrival.

No specific form is required to meet the immunization requirement. Any records that include the dates of vaccination/testing along with a doctor/clinic signature or stamp will be accepted. Lab reports must include a numerical value and reference range. In the case you are missing an immunization, the Health Center might contact you with instructions to complete the missing requirements at the Student Health Center.

Authorizations
Blanket liability waiver for your participation in on-campus and off-campus activities associated with the USC Summer Programs.

Summer Programs Appendix Forms which include:

  • Letter of Understanding
    An agreement between you, the parent(s) or guardian(s), and the USC Summer Programs Office on the rules and expectations of students enrolled in the program. Outlines specific expectations USC has of its youth student participants.
  • Permit to Register
    Required document to register for a course at the University of Southern California.
  • Misconduct Adjudication Procedure
    Outlines the procedure our program follows in the case of student or misconduct.
  • Domestic Participant Tax Credit
    Required by the University of Southern California so that qualified students can receive federal education tax credits for qualified education expenses.

Emergency Health Release Form
Permission that is required in the case of an emergency that is outside the scope of the USC Health Center and necessitates a visit to an emergency room. This form also gives families the opportunity to request USC Health Insurance for the student, if needed.

Commuter Participant – Permission to Stay on Campus
Permission for commuter students to stay on campus after class.
